| When the clerk gets DA Form 5988-E, the clerk must
enter each new uncorrected fault into the ULLS-G database. In this example, the clerk must
log on to the computer and enter the new faults for D2. The clerk must enter the maintenance
faults before he can request the parts.

To update the ULLS-G database, from the
ULLS MAIN MENU the
clerk selects OPERATIONAL PROCESSES [I].
From the MAINT. OPERATIONAL PROCESS menu,
he selects MAINTENANCE FAULTS [3], and
from that menu he selects ADD FAULTS. The
clerk types in D2, which is the vehicle administrative number,
and presses <Enter>. A blank add
fault screen will appear. The clerk then adds the data from the DA Form 5988-E. |

| Here is the data the clerk would add to the
ULLS-G database for one of the faults shown on the DA Form 5988-E.

The codes across the lower part of the screen are only used as
default data for a work order request. There are too many codes to memorize. You can get the
codes from the End User Manual (EUM) or by placing the cursor in the appropriate block and
pressing the <F1> key. This works
for any data field in ULLS-G. The information is entered by the clerk. |

| Here is the data the clerk would add for the safety
fault on the DA Form 5988-E.

| ULLS-G will not allow a part to be ordered for
an "E" status symbol fault.
This is an ULLS-G problem that is scheduled to be fixed. Until this problem is fixed, the
clerk must enter two faults for the headlights: one for the actual fault status symbol, and
another one so the part can be requested.

| After the clerk adds this remaining fault
from the DA Form 5988-E, he can then request the parts.

Now all of the faults reported by the crew have been
added to the database for our example. When the DA Form 5988-E is updated, it will look like
the entries below, but the clerk must request the parts before the form is updated. There
have been some changes from what the crew reported and what the clerk has entered in the
database.

Notice that the status symbol for the headlight has changed from
a "/" reported by the crew to an "E" status symbol. ULLS-G uses an
"X"
for NMC faults
and an "E" for safety
deadline faults. |

Also, notice that the headlight fault
is entered twice. It is entered once for the correct status symbol and once to request
the part. This should change when the ULLS-G program is fixed. |
 | The fault item number has also changed. ULLS-G
automatically assigns an indexing number when the fault is added. It has nothing to do
with TM item
numbers. |

Now that the clerk has entered all the faults in
ULLS-G,
he must now request repair parts. There are three methods of obtaining repair parts.

Quick supply store (QSS). |

Request for issue. |
 | Repairable exchange (RX). |

| To request the repair parts, the clerk goes back to the
ULLS MAIN MENU and selects REQUEST PROCESS [A], then REQUEST FOR ISSUE [1].

The IPD is determined by the unit Force Activity Designator (FAD)
and the part urgency of need. The FAD is the timeframe in which a unit will be expected to
deploy. The unit's FAD is already entered into the ULLS-G database and becomes default
information.
The urgency of need designator (UND) is either A, B, or C.
The clerk then uses the priority designator table to determine
the IPD for each
requested part. This table is in DA Pam 710-2-1, in the Unit Supply Update. For this lesson,
our unit, Company D, 1/10 Armor, has an FAD of II. The clerk must request three parts for
vehicle D2.

Determine the IPD of each part.
 |
I tem number 2 has a fault status of "X" with the TM item number circled
to show it is NMC. The UND for this
part is "A." This means the
clerk must use IPD 02 and request the part as a not mission capable-supply (NMCS) item.
This will make an entry on the deadline file. The IPD will be entered into the priority
block in the Request For Issue option in ULLS-G which will be explained later in this
lesson. |

Item number 8 has a fault status of "/," but it is a safety
deficiency. The unit SOP will specify whether to use
UND "A" or "B." Most units use UND "B." The clerk would use IPD 05
for UND "B." Regardless of
the UND, the part is not requested as
an NMCS part. The vehicle can do its combat mission with this fault. |
 | Item number 10 is a routine repair and has a UND "C." The clerk must use IPD 12. |
